Change Hostname - Question for data

Hi guys!

One of our servers will be replaced with a new machine and the hostname and IP Address will be retained. (Only the machine will be replaced)
However, we will still monitor the old machine with Hobbit.

My question is that is the data from the old machine will be retained for its new hostname?
I think there's a command with hobbit on how to change the hostname to monitor but can someone please advise me for the procedure?
