Microsoft's Licensing page for System Center Configuration Manager (SCCM), the new SMS, is http://www.microsoft.com/systemcenter/configurationmanager/en/us/pricing-licensing.aspx. Your pricing may differ if you have an Enterprise Agreement and if you choose to place Software Assurance on the purchase or not. We have a single server with sql server bundled. We use the middle tier management license. It services about 100 servers and 900 desktops.
